History & Purpose

he San Francisco Association for Docket, Calendar and Court Services was formed in 1984 to provide a forum for persons involved in docketing, calendaring, or court services functions. The primary purposes of the organization are:

1n To facilitate communication between court clerks and members to promote a better understanding of, and compliance with, court rules and procedures; and
n To promote the exchange of information between members regarding court rules and procedures.
The Association currently has over 275 members from small, medium and large Bay Area law firms and other legal organizations.
 

New Civil Cover Sheet (form JS-44)

Please be advised that effective January 1, 2005, when you file a new civil case, the U.S. District Court will require the use of a new civil cover sheet (form JS-44). The civil cover sheet includes five new nature of suit categories: 196: Franchise - Civil complaints filed in connection with a dispute in a franchise agreement, filed under 15 USC 2805, 16 CFR Part 435, 42 USC 532, and 47 USC 542. 445: Americans with Disabilities - Employment - Discrimination against an employee with disabilities of any type in the work place, filed under 42 USC 12117. 446: Americans with Disabilities - Other - Discrimination against an individual with disabilities in areas other than employment, filed under 42 USC 12133 (exclusion or discrimination in provision of services, programs or activities of a public entity) or 42 USC 12188 (public accommodations). 480: Consumer Credit - Civil complaints filed under the Fair Credit Reporting Act, 15 USC 1681n or 15 USC 1681o, and the Fair Debt Collection Practices Act, 15 USC 1692k. 490: Cable/Satellite TV - Civil complaints filed involving unauthorized reception of cable/satellite TV service under 47 USC 553 (unauthorized reception of cable/satellite TV) or 47 USC 605(e)(3) (unauthorized use or publication of a communication).
